I can tell you in regular season that if it’s blatantly obvious, the manager is fined after the fact.

Trust me on this b/c it happened to us this year. Pinch hitter called into the game from the BP (third cather being used in the BP), had to come through the tunnel, stopped to take a couple of practice swings, finally made it to the plate.
Jerry M. was fined two days later for that.

I say game 6 and 7

we just have our 5 starters pitch all 18+ innings. Kaz has stuff to come out of the bullpen as does Santana you better believe guys Lackey is available for 3 innings in Game 7.
